New Client Forms

Below are forms for first-time clients.  If you are able to print, fill it out and bring it with you to your first session,  you will be ready to be seen as soon as possible.  If not, we will have you complete this upon arrival.

Each subsequent visit we provide you with the opportunity to update any relevant information that may have changed since your last visit. As with other healthcare providers, any information you provide us is protected under the HIPAA privacy rule.

Starting Your Session

Your massage therapist will then take you to a peaceful treatment room where a comfortable massage table, dressed with a warm blanket and crisp clean sheets awaits. A small space for your personal belongings and a chair are provided for your use. Your massage therapist will leave the room and wait outside while you disrobe to your level of comfort.  Massage etiquette varies – some clients prefer to completely disrobe, while some will remain partially clothed. Either option is fine. Once disrobed, you will lie on the massage table, under the top sheet and blanket. Your massage therapist will give you a few minutes for this process and will knock on the door to ask if you are ready before entering your room.

You will always be draped with the top sheet and blanket during your massage session. The etiquette your massage therapist will follow is to only uncover the part of the body they are working on, ensuring that your modesty is respected at all times.

You can expect a tranquil and comfortable environment during your massage. Your therapist will take a few moments to adjust any pillows, bolsters, coverings, music or room temperature to be sure you feel secure and comfortable.

During Your Massage

Before beginning the treatment, your therapist will perform a brief assessment – based on the reason you came in for massage therapy. For example, if your primary reason(s) for seeking massage therapy is to relax, then our assessment might center around a few questions with regard to your current stress level. If your primary reason is related to pain, for example low back pain or wrist pain, then the assessment might center around range of motion tests to determine those structures or muscles possibly involved in your cycle of pain or tension.

Your therapist will explain a massage approach based on your needs and share areas of your body they intend to focus on before asking your permission to proceed. If you are new to massage, or just new to us, they will establish a “comfort scale” that helps you communicate to us a comfortable degree amount of pressure to use. As always, you have control at all times and can communicate an increase or decrease in pressure as desired to make your treatment most suitable for you.  A massage therapy session can last anywhere from 30 – 90 minutes depending on how much time you choose to schedule.

Ensuring Your Wellness

When your massage therapy session is complete, your therapist will leave the room so you may re-dress. Once redressed, they will return and take a few minutes to discuss your treatment. This is a great time to ask any questions you might have. You will also be provided a prospective on your treatment as well as  “home care tips” if appropriate.

Home care tips are stretches, exercise or movement modifications we believe are suited to supporting the work you just received. For example, if you came in with neck pain and in the course of our work in we determined specific muscles that need stretching we would explain those stretches to you. Then we take you back to the front area. At this time, you may book another appointment or call at a later date.
